Rules and Eligibility

• Winner will receive payment up to $500 USD towards the amount they paid for their IVF consultation fee.

• Must register online via the form above.

• Entry form must be completed in its entirety.

• Pricing sheet for IVF cost needs to be uploaded with entry form.

• Must have had an IVF consultation in the last 12 months.

• Only one entry form per person. Contest open for US residents only.

• Valid pricing sheet must have pricing for a standard IVF cycle.

• Winner will be selected randomly.

• A winner will be announced on June 1st.

• IVF Options reserves the right to discontinue the contest at any time.
