Is IVF Covered in Maryland?

In Maryland insurers including HMOs are required to cover the outpatient costs of In Vitro Fertilization (IVF) treatment. In order to qualify for the mandate the patient must provide documented evidence that they have been unable to conceive for two years.

The mandate includes coverage for same sex couples who have used donated sperm or eggs during an IVF cycle. Same sex couples must document at least six failed IUI (artificial insemination) attempts.

Insurers may limit coverage to three IVF cycles with a lifetime cap of $100,000. Religious organization may be exempt if treatment conflicts with their beliefs.
